The Treatment of Minor - 3rd Party Consent form should be completed by the parent of any student who is not yet 18 years old at the time they arrive on campus. By having Treatment of Minor - 3rd Party Consent on file, students who are still 17 years of age or younger may seek treatment at Student Health Center without significant delays.
